The pressures are real
More patient needs. The same hours in the day.
Rural clinics deserve practical support that respects their time, their teams and their relationships with patients.
01 / FOLLOW-UP GAPS
What happens after the patient goes home?
Distance, missed follow-ups and changing symptoms can make chronic care harder to manage between appointments.
A connected approach: scheduled outreach and, when appropriate, home readings to give the care team a clearer picture.
02 / STAFF CAPACITY
Your team already wears enough hats.
Calls, care coordination and routine follow-up compete with the patients sitting in your waiting room.
Support beyond your walls: a dedicated care-navigation team can handle agreed activities without your clinic building a new department.
03 / FINANCIAL SUSTAINABILITY
Support your patients and your practice.
Rising operating demands make it important to evaluate how care management can support your clinic’s financial health.
A revenue opportunity: eligible APCM, CCM and RPM services can generate additional reimbursement. We’ll review documentation, expected collections and program costs to assess the potential net benefit.
Three services. A clinic-specific approach.
Choose the right starting point.
Explore one service or a clinically appropriate combination. We’ll help you evaluate what fits your patients and existing setup.
APCM
Advanced Primary Care Management
Comprehensive, ongoing primary care management through a monthly service bundle.
Services depend on patient eligibility, medical necessity, consent and applicable payer requirements. APCM and CCM are generally alternative billing approaches for the same patient and month; they are not automatically additive. Any RPM combination requires a separate eligibility and billing review. Monitoring is not an emergency-response service.
Designed around your practice
Extend your team. Keep your clinical direction.
The proposed model uses HealthSnap technology and care-management support to help your clinic put a workable program in place.
No fee to host the platform under the proposed offering. We’ll review service pricing, device costs, integration terms and any other charges before you commit.
Support without building an in-house department
Care-navigation support, outreach and documentation can reduce the need to recruit a separate internal team. Your clinic retains clinical oversight and agreed responsibilities.
Potential for additional practice revenue
Eligible care-management services may create a recurring revenue stream while supporting care between visits. We’ll evaluate your patient population, payer requirements, expected collections and full program costs to estimate the potential net financial benefit. Revenue and profit increases are not guaranteed.
Work with the setup you already use
The supplied HealthSnap overview describes EHR integration and device logistics support. We’ll confirm your EHR compatibility, staff touchpoints and onboarding needs during the assessment.
Why this conversation matters now
Rural care is moving forward. Plan your next step.
Rural Health Transformation Program
The Medicare & You 2027 excerpt highlights a national effort to strengthen rural health systems, expand access, improve facilities and modernize health technology.
CMS describes a $50 billion program for approved states over fiscal years 2026–2030, including support for chronic disease management and technology-enabled care.
For your clinic: this is a timely reason to assess care-management capacity and technology readiness. Contact your state health department about local initiatives and eligibility.
Your supplied Medicare & You 2027 excerpt also introduces ACCESS—Advancing Chronic Care with Effective, Scalable Solutions—a voluntary model for people in Original Medicare.
The CMS model explores technology-supported chronic care and payment tied to health outcomes. It is distinct from routine APCM, CCM and RPM billing.
For your clinic: evaluate how remote care and care coordination could support your patients today. ACCESS participation requires a separate review.
These federal initiatives provide context; they do not establish funding eligibility, reimbursement, endorsement or ACCESS participation for Royal Orleans, HealthSnap or your clinic. Scheduling a consultation does not enroll a clinic in a government program.
No. Start with the service that best fits your patient population and clinic workflow. Eligibility, existing vendor arrangements and billing overlap determine which combinations are appropriate.
Will we need to hire more staff?
The proposed model brings outsourced care-management support to reduce the need for a separate in-house team. Your clinic still needs a point of contact, clinical oversight and participation in onboarding and escalation. We’ll map those responsibilities before launch.
Does “no hosting fee” mean everything is free?
No. The no-hosting-fee offering refers to platform hosting, not a promise that all clinical services, devices, integrations or implementation activities are free. A written proposal should identify every applicable charge and responsibility. Patient cost-sharing may also apply.
Will this work with our current EHR?
Compatibility must be confirmed for your particular EHR and configuration. The consultation is the starting point for reviewing interfaces, documentation flow, access and implementation needs.
Does the Rural Health Transformation Program pay for this?
Not automatically. States administer their approved initiatives, and any funding depends on applicable state opportunities and requirements. Contact your state health department. Our consultation does not secure or guarantee a grant.
Can you guarantee more revenue or fewer hospitalizations?
No. Financial and clinical results vary. Any proposal should evaluate patient eligibility, actual services, payer rules, collections, program costs and the clinic’s responsibilities before estimating a potential return.
